Grey Face Rescue Dog Surrender Form

Please, take a moment to read ALL details regarding the surrendering process at Grey Face Rescue.

There are times when an owner can no longer care for their pet. Our Surrender Form provides information we need in order to determine if we can assist with the re-homing of a dog. Although we would love to help all pets, we need to assess our current foster home availability, among other factors, before making a decision.

 Please allow up to one week for a response from our team.

If you do decide to surrender your dog, our process is designed to find the most compatible foster home for the dogs that come into our rescue.

  1. Our team will contact you for photos, updated veterinary records and any additional information needed.

  2. If a foster is found, our Intake Manager will send surrender instructions.

  3. Prior to surrender appointment being scheduled, you will be required to send veterinary records and pay the surrender fee.

  4. Upon arrival, surrender appointments are approximately 30-minutes long. We ask that you say your goodbyes prior to arrival. You are expected to provide a full, unopened bag of food and all of the dog's supplies: food, bowls, leashes, collars, beds, treats, etc., to help ease the transition.

Dog we DO NOT accept into rescue:

  • Animals that have bitten another animal or person under any circumstance.

  • Dogs that are not friendly with other dogs.

Unvaccinated and/or unaltered dogs; $150 Surrender Fee
Vaccinated & altered dogs; $100 Surrender Fee
