GREY FACE RESCUE ADOPTION CONTRACT

This Adoption Contract (hereinafter “The Contract”) is made on the aforementioned date, by and between Grey Face Rescue, Inc. (hereinafter “Grey Face”) and Adopter, identified above. Grey Face hereby delivers to Adopter, and Adopter hereby takes as a domesticated, indoor pet, the pet listed above. The parties hereby agree to the following:

1. Standard of Care for The Pet

a. Adopter shall accept and agree to keep The Pet as a household companion, not as an outside pet, and will provide The Pet with a safe environment, humane treatment, fresh water, proper food, exercise, and indoor-household shelter. Adopter agrees The Pet shall sleep exclusively indoors, and shall be protected from the elements, with the exception of outdoor activities which may necessitate outdoor sleeping. In this case, Adopter is still responsible to provide adequate shelter (i.e. a tent) Adopter agrees to prioritize the health condition and abilities of The Pet before engaging in said activities. The Pet shall not be allowed off of Adopter’s property unless accompanied by an adult who is experienced in handling The Pet. The Pet shall never be allowed to roam free outdoors while unsupervised.

b. If for any reason, at any point during The Pet’s lifetime, Adopter is unable to provide care for The Pet, Adopter agrees to return The Pet to Grey Face Rescue. Adopter shall not give or sell The Pet to any third party – even a family member or close friend – without the express prior written consent of Grey Face. If The Pet is returned to Grey Face, no refunds are given for adoption fees or any expenses Adopter incurred relating to The Pet while in Adopter’s care.

c. Adopter is asked to deliver The Pet to the Grey Face Headquarters located in St. Joe no sooner than 72 hours from notifying our Adoption team to return. If a foster is not found within 72 hours, you may be asked to care for the animal longer until a foster home is found.

d. Adopter agrees to provide reasonable, timely, and proper veterinary care for The Pet, which includes but is not limited to, routine vaccinations, heartworm testing, monthly heartworm preventative medication, and flea/tick protection and prevention.

e. Adopter acknowledges that The Pet’s health and history are accurately represented to the best of Grey Face’s knowledge. Adopter understands that Grey Face makes no representations or warranties as to the temperament, mental disposition, or physical condition of The Pet.

2. Pre-Adoption

a. Adopter represents that he/she has never been subject to legal action for cruelty or neglect to any animal. Adopter further represents he/she has never owned an animal which has been confiscated by any animal control or humane organization for violations of local or state animal control regulations, animal care regulations, or animal adoption agreements.

b. Adopter certifies all family and household members have consented to the adoption of The Pet and that all family and household members will likewise abide by the terms of this Contract.

c. In the event Adopter is a tenant, Adopter must provide the landlord or property owner’s written permission consenting to adoption of The Pet.

d. Adopter hereby acknowledges that he/she is aware:

i. That all pets respond to human actions differently, and unpredictably;

ii. That The Pet’s behavior may change after it becomes accustomed to a home or new environment; and

iii. Grey Face makes no representations as to The Pet’s temperament, health, or mental disposition.

e. Adopter acknowledges he/she has fourteen (14) days from the above date to return The Pet to Grey Face for a refund of the Adoption Fee identified above. After that 14-day period has passed, Adopter acknowledges there will be no refund of the above-referenced Adoption Fee.

3. Post Adoption

a. Adopter shall allow a 3-day period of time the pet enters the home to adjust and become familiar with its new surroundings.

b. Upon reasonable notice, Adopter grants Grey Face the right to enter Adopter’s premises post-adoption for the purpose of checking on the health and general welfare of The Pet, verifying The Pet has been spayed/neutered, and to ensure Adopter is in compliance with all terms and conditions of this Contract.

c. Adopter agrees to meet all local licensing requirements for The Pet. Adopter further agrees The Pet shall wear proper identification at all times; at a minimum, proper identification includes a municipal license tag, and an ID tag with The Pet’s name and Adopter’s telephone number, both of which must be clearly identifiable at all times.

d. Adopter agrees Grey Face shall have the right to immediate possession of The Pet if, in the exclusive judgment of Grey Face, The Pet is receiving inadequate care, is being housed improperly, or is being improperly handled.

e. Adopter understands that he/she is encouraged to call Grey Face Rescue at any time for behavioral counseling in the event that any questions or issues arise with The Pet. No question or issue is too small! Please call us and let us help!

f. Adopter acknowledges he/she has been provided a copy of this Contract.

g. The adopter will participate in ‘Adoption Updates’ and respond to future emails.

h. If a surgery is scheduled for the animal adopted from Grey Face Rescue post-adoption, Grey Face Rescue is responsible for all costs associated with the surgery scheduled which includes medications and follow-up appointments. Grey Face Rescue will not be responsible for services, testing or care outside of the surgery (nail trims, heartworm tests, grooming, mass aspirations) specified prior to adoption.

4. Veterinary Care

a. Grey Face Rescue does its absolute best to address all and any veterinary concerns each animal faces that come to Grey Face Rescue but unfortunately, because they are seniors, we can not address them all.

b. We work with partner veterinary partners that prepare a veterinary plan for each animal and needs they feel need to be addressed immediately.

c. Grey Face Rescue ONLY performs dental procedures on animals with Stage 4 dental disease diagnosed by our partner veterinary clinics.

d. Grey Face Rescue is not liable for any veterinary needs diagnosed following the adoption.

e. If you animal is diagnosed with heart worm approximately 2-months post-adoption, Grey Face Rescue will cover the expense of heart worm treatment. Any positive diagnosis past 2-months will not be covered by Grey Face Rescue.

f. If you are ever facing difficulties with veterinary care, please reach out to our team as we can assist and/or recommend clinics, treatments and other resources.

5. Release of Liability

a. Adopter adopts The Pet at their own risk. Adopter, and Adopter’s heirs, beneficiaries, and representatives, voluntarily and knowingly execute this document and expressly waive any and all rights, claims, causes of action, including, without limitation, those involving bodily injury to any person or property. Adopter agrees to indemnify, defend, and hold Grey Face Rescue, Inc., its officers, directors, agents, and employees harmless from and against any and all liability, damage, loss, costs and expense (including attorney’s fees), incurred as a result of any claim, demand, or cause of action brought against Grey Face, its officers, directors, agents, or employees – jointly or individually – for bodily injury or property damages suffered as a result of adoption of The Pet

b. Once adopted, Grey Face is not responsible for any costs relating to The Pet. If a surgery is scheduled for the animal adopted from Grey Face Rescue post-adoption, Grey Face Rescue is responsible for all costs associated with the surgery scheduled which includes medications and follow-up appointments. Grey Face Rescue will not be responsible for services, testing or care outside of the surgery (nail trims, heartworm tests, grooming, mass aspirations) specified prior to adoption.

c. Adopter acknowledges Grey Face makes no warranties, representations, or guarantees regarding The Pet’s physical health, personality, or mental disposition.

d. This Contract is subject to Minnesota Law.
